Lumbago post epidural

[Objective] To observe the effect of Capsaicin injection for prevention of lumbago after epidural anesthesia. [Methods] Ninety patients had undergone lower abdomen or lower limb operations in epidural anesthesia were randomly divided into three groups, 30 cases for each group. In group Ⅰthe bevel of the needle was inserted vertically along the spinal axis (vertical insertion method), while in groupⅡthe bevel of the needle was inserted horizontally along the spinal axis and rotated to the demanded direction later (horizontal insertion method), group Ⅲ was also given vertical insertion method, and injected 3~5 ml Capsaicin (0.1%) around puncture position after epidural anesthesia. Then all patients were given Lidocaine (0.66%) and Naropin (0.66%) epidurally. Lumbago severity and incidence were examined in respectively seven days after operation. [Results] The incidence of lumbago in three groups were 46.7%, 20% and 16.7% respectively. The severity and incidence of lumbago of group Ⅱ were significantly lower than that of group Ⅰ(P 0.05). The severity and incidence of lumbago of group Ⅲ were significantly lower than that of groupⅠ(P 0.05). [Conclusion] Injected Capsaicin (0.1%) around puncture position after epidural anesthesia can reduce the severity and incidence of lumbago.

The clinical observation of prevention of postpunctural lumbago after epidural anesthesia

Objective To study the causes and prevention of postpunctural lumbago after epidural anesthesia. Methods Ninety-six abdominal tumor patients scheduled for operation in epidural anesthesia were randomly divided into three groups, 32 cases for each group. In group Ⅰthe bevel of the needle was inserted vertically along the spinal axis(vertical insertion method), while in group Ⅱthe bevel of the needle was inserted horizontally along the spinal axis and rotated to the demanded direction later ( horizontal insertion method). Group Ⅰand Ⅱreceived 15 mL bupivacaine (0.75%) and 2 mL normal saline epidurally. GroupⅢ was also given horizontal insertion method,and received 15 mL bupivacaine (0.75%) and 2 mL dexamethasone (10mg). Then, all patients were given 5~7 mL bupivacaine epidurally. Postpunctural lumbago severity and incidence were examined. Results The incidences of postpunctural lumbago in three groups were 43.8% , 21.9 % and 3.1%, respectively five days after operation. The severity and incidence of postpunctural lumbago of group Ⅱwas significantly lower than that of group Ⅰ(P0.05), the severity and incidence of postpunctural lumbago of group Ⅲwas significantly lower than that of group Ⅱ (P0.05). Conclusion Horizontal insertion method and addition of dexamethasone to local anesthetic could reduce the severity and incidence of postpunctural lumbago.
